What is the treatment of anal fissure (a cut or wound in anus or rectum) besides surgery.?

I have anal fissure for last two years. I feel severe pain and burning during and after bowel movement which remains from few minutes to few hours sometimes. I have applied several creams, ointments, herbal ointments but no benefit. I have also chronic constipation. I also eat vegetables, drink lots of water, take fibre but no benefit. Please help I shall pray for this forever.

I feel your pain- been there and it’s horrible. the constipation will unfortunately make you strain for bowel movements and will not help the area heal. Take laxatives to soften your stools because as long as the constipation is there the fissure will not get better. Keep using creams- prep H is a good one and can help soothe the area. I found baths helped a bit as did using lubricant when I had a bowel movement. I also used cold compresses on the affected area. Cold worked for me but I have heard that heat can also help. However, I would recommend seeing a doc. 2 years is a long time to suffer this and it must make you miserable- I remember crying over the pain sometimes and it really got me down.

The three most common alternatives for non-surgical procedures for treatment of hemorrhoids are rubber band ligation, infrared photocoagulation and sclerotherapy.

Rubber Band Ligation. A rubber band is placed around the base of the hemorrhoid inside the rectum. The band cuts off circulation, and the hemorrhoid withers away within 1-2 weeks, leaving an ulcer that later fibroses.

Rubber band ligation is the most widely used treatment for grade II and grade III hemorrhoids and is the standard to which other methods are compared. A band ligature is passed through an anoscope and placed on the rectal mucosa proximal to the dentate line.

“Pros”

• High success rate. Up to 80% of people who had this procedure said their symptoms improved
• Great long-term efficacy
• Repeat treatment for recurring symptoms is rarely needed

“Cons”

• Severe pain that sometimes does not respond to pain killers
• Bleeding from the anus
• Necrotizing pelvic sepsis is a rare, but serious, complication of rubber band ligation. It occurs 1-2 weeks after ligation, frequently in immune compromised patients, and requires prompt surgical debridement

Laser or Infrared Coagulation. These methods involve the destruction of hemorrhoids with laser or infrared light. Coagulation causes the hemorrhoidal tissue to harden and degenerate, and to form scar tissue as the area heals. Coagulation is generally effective for grade I, grade II, and some grade III hemorrhoids.

“Pros”

• Fewer and less severe complications than with rubber band ligation
• Simple, convenient and fast
• No serious complications

“Cons”

• Recurrence rates may be higher than with rubber band ligation

Sclerotherapy. A chemical solution is injected around the blood vessel to shrink the hemorrhoid.

I.S. is an alternative to rubber band ligation. Injection of sclerosing chemical substances like phenol in oil, polidocanol, concentrated NaCl solution, into the mucosa at the haemorroid’s origin, leads to tissue necrosis.

Rare major complications include pelvic infection and impotence, as result of incorrectly sited injection.

• Fewer and less severe complications than with rubber band ligation

“Cons”

• Recurrence rates may be higher than with rubber band ligation
• Urological complications
• Not suitable for anterior haemorrhoids

Hemorrhoidectomy (surgery). Occasionally, extensive or severe internal or external hemorrhoids may require removal by surgery known as hemorrhoidectomy.

“Pros”

• Highest success rate
• Best therapy for fourth degree hemorrhoids

“Cons”

• Highest rate of pain and complications. The complication rate is about 10-20%

OTC remedies. Over-the-counter creams and suppositories (e.g. Preparation H, Neo Healar) can help with the symptoms, but unlikely shrink the hemorrhoids.

“Pros”

• Can help with the symptoms like pain, swelling, itching
• No risks of treatment-related complications

“Cons”

• Unlikely shrink the hemorrhoids

How Can I know if my dog has Irritable Bowel Syndrome or Inflammatory Bowel Disease?

She has diarrhea, and has had it for a long time. she doesn’t have worms or enzyme insuffiecy. And how can I help her? I’ve already tried rice, chicken, keopectate, different kinds of dog food, and yogurt.
I’ve taken her to the vet alot! and have spent alot in tests and medication. the vet says its probably irritable bowel syndrome and that we should be careful about stressing her out, like don’t get her to excited.

Take the dog to the vet. Diarrhea continuing for more than about 24 hours should be evaluated. It could be all sorts of things wrong with her, from parasites to food allergies (I know a dog that is Allergic to beef, chicken, turkey, duck, rice, wheat, soy, corn, barley and millet), to something even more serious. The only way to tell and to fix the issue is to have your vet examine your dog.

It’s been said that if it wasn’t for haemorrhoids, Napoleon Bonaparte might have won the Battle of Waterloo.

He suffered a lot from haemorrhoids and that morning was very constipated and in great pain. So he left the battlefield and sat in a bath all morning to relieve the pain.

In the afternoon he rejoined his army and began the famous battle.

Unfortunately for him conditions had changed to advantage the British and he lost – which must have been an even bigger pain in the backside for him.

Haemorrhoids have been around throughout history, back to the days of Hippocrates. It is believed he created the word to describe the flow of blood from the veins of the anus.

The word haemorrhoid is derived from the Greek ‘haeme” (blood) and “rhoos” (flowing). It is only used in the plural.

But it’s a pain of a word as well as a painful condition.

I’m using the old fashion version of its spelling – haemorrhoids – because I’m an old fashioned classical type.

Many people nowadays spell it as hemorrhoids. There’s even an increased usage of hemroids.

But did you know that this silly word that only comes in the plural has at least 12 other versions of spelling? My oath that is a wacko word.

There is a way to avoid the word. It was developed over the years by working class people to describe the condition, probably because they could not spell haemorrhoids. They called it “piles”.

Nowadays piles is accepted as meaning the same thing as haemorrhoids or whichever way you spell it.

So the easy way is to say piles, but the pain of the condition is still there.

Interesting though that the word piles also stays only in the plural.
